Instructions: The chest x-ray or CT scan provided does not indicate if it was read by a Qualified Physician, or the physician who read the chest x-ray or CT scan was not board-certified at the time of the reading. Please provide documentation of the physician's certification or provide a chest x-ray or CT scan that was read by a Qualified Physician. The physician must be board-certified at the time of the reading.

Detail Text: The chest x-ray or CT report submitted does not indicate if the chest x-ray or CT scan was read by a board-certified physician (defined in footnote 3 in the TDP) or the physician who read the chest x-ray or CT scan was not board certified at the time of the reading. Please provide documentation of the physician's certification or provide a chest x-ray or CT scan report that was read by a board-certified physician and supports the alleged injury. The physician must be board certified at the time of the reading.
