Detail Text: If you choose to file a claim for Severe Asbestosis (Disease Level IV) or Asbestosis/Pleural Disease (Disease Level III), please submit a qualifying Pulmonary Function Test (PFT) report, including at least the FVC and FEV1/FVC ratio and/or TLC scores that meet the criteria established in the TDP.
